Stroke Association: Support Coordinator - South West Essex

If you or someone you know has had a stroke (and is over the age of 18), we are here with help and support from day one and for as long as you need us. Our Stroke Recovery Service offers practical information, guidance and support after a stroke.

A Stroke Association Support Coordinator will contact and they will work with you to identify your priorities and what is important to you so that we can best support you to rebuild your life after stroke.  We can also connect you to other people affected by stroke who understand what you are going through.

How are services delivered?

The service is free and open to anyone, over the age of 18, affected by stroke who is registered with a GP in Basildon, Brentwood or Thurrock.

You can be referred to the service by a health professional or community organisation or you can self-refer. You will be contacted by telephone to assess your needs and discuss support.
