EPUT: Carers' Intensive Support Team - Thurrock
EPUT: Carers Intensive Support Team provide a range of services to support vulnerable and ageing carers living in Thurrock. This includes: being a tangible link between health and care services, identifying and supporting carers experiencing carer burden to taking action before breakdown occurs, assisting with carers emergency and contingency planning, supporting carers in crisis and providing personalised health and wellbeing checks to help carers remain well to manage caring responsibilities. Referrals for this service are only accepted from professionals (no self-referrals).

We support Carers who live in Thurrock plus one of the following:
- Carers of any age supporting a person over 18 with suspected or confirmed dementia
- Carers aged 65 or over supporting a person over 65 with frailty and long term conditions
- Carers aged 65 or over supporting a person over 65 with mental health concerns
